Blanchfield Army Community Hospital to offer school, sports physicals this summer

Blanchfield Army Community Hospital (BACH)Fort Campbell, KY – The Blanchfield Army Community Hospital (BACH) patient-centered medical home teams are offering extended-hours clinics in the afternoon for BACH-enrolled TRICARE Prime children to receive school and sports physicals this summer.

If your child had a school or sports physical completed at one of BACH’s medical homes within the last 12 months and has had no medical changes, it is not necessary to receive another physical. BACH medical home patients who do not need a new physical may contact their care team to have the required school forms completed.

Paperwork may take up to three business days to complete.

A school and sports physical is typically required for:

  • Any child entering Fort Campbell, Christian County, or Clarksville-Montgomery County schools for the first time
  • Any child participating in school organized sports
  • Any child participating in Fort Campbell’s organized youth sports programs
  • Any child who enrolls in the 6th grade in Christian County or Fort Campbell Schools
  • Any child who is entering the 7th grade in Tennessee

School and sports physical afternoons will be hosted separately by each medical home from 1:00pm to 5:00pm on the following dates:

June 20th – Air Assault Family Medical Home

June 20th – Byrd Family Medical Home

June 27th – Screaming Eagle Medical Home

July 11th – Young Eagle Medical Home

July 25th – Screaming Eagle Medical Home

August 8th – Byrd Family Medical Home

August 22nd – Air Assault Family Medical Home

August 22nd – Young Eagle Medical Home

If the school and sports physical date offered at your child’s assigned medical home is not a convenient date and time, parents may request to schedule their child’s appointment on one of the other dates in a different medical home. Children should wear shorts and a short sleeved shirt to facilitate the examination.

“We are encouraging all parents to call now to get their child(ren’s) school and sports physicals scheduled ,” said Col. Troy Prairie, chief of BACH Primary Care. Parents are encouraged to download the appropriate sports physical forms required by the child’s school district and complete the forms prior to arriving at the physical appointment.

“There are a number of pages and questions and completing the paperwork ahead of time will help ease the process and reduce wait time. The forms are different for Tennessee, Kentucky and on post so its important to make sure you have the correct forms,” said Prairie.

Parents of patients with more complex or complicated conditions, or who may require additional time should schedule a separate visit with their primary care provider during a normal appointment time. Please specify your child’s individual needs when making their school and sports physical appointment.

If other medical concerns arise during the school or sports physical examination, a follow-up appointment should be scheduled with your primary care manager in order to ensure proper time and attention is devoted to the medical need. Routine physical exams are valid for one year.

Parents of patients assigned to a BACH medical home may schedule a school and sports physical by calling the appointment line at 270.798.4677 or 931.431.4677, or by emailing your care team using the TOL Secure Email Messaging.

To help care teams make preparations for your visit when calling the appointment line or sending a TRICARE Online (TOL) secure message, please inform the team whether your child will attend a school in Kentucky or Tennessee, or another state during the 2019-20 school year. If you are leaving the Kentucky or Tennessee areas before school begins, please download the appropriate school form to take to your child’s appointment.

Patients unable to attend a scheduled school or sports physical appointment should call the BACH appointment line to cancel at least two hours prior to the appointment.
