Screaming Eagle Medical Home expands enrollment

Community Based Medical HomesFort Campbell, KY – The Screaming Eagle Medical Home, the community based medical clinic operated by Blanchfield Army Community Hospital (BACH), expands eligibility to now include three Clarksville and Adams, Tennessee zip codes. The clinic is located in Medical Office Building One, 647 Dunlop Lane, Suite 301, next to Gateway Medical Center.

Active Duty Family members living in the 37043 zip code should have already received letters offering them the opportunity to enroll to the Screaming Eagle Medical Home. Letters and enrollment instructions are now being mailed to active duty Family members living in the 37010 and 37040 zip codes. Some individuals living in the 37040 zip code may not receive their letters until early March to allow for phased enrollment success.

If you live in the 37043 or 37010 zip code but did not receive a letter and would like to enroll in the Screaming Eagle Medical Home, please go to Fort Campbell’s TRICARE Service Center from 8:00am to 5:00pm, Monday through Friday to enroll. The TRICARE Service Center is located in Bldg. 2525, 22nd and Indiana Avenue.

Although the Screaming Eagle Medical Home is available to active duty Family members living in these zip codes, patients may choose to remain enrolled to their current healthcare provider.  If patients living in these three zip codes prefer to change their enrollment to the Screaming Eagle Medical Home, they are asked to complete and sign the primary care manager (PCM) change request form received by mail and submit through one of the following processes:

  1. Scan and e-mail it to Kenneth.giffin@healthnet.com.
  2. Fax to 931-431-6168.
  3. Mail to the TRICARE Service Center, 2525 22nd Street, P.O. Box 444, Fort Campbell, KY  42223.
  4. Take the form to Fort Campbell’s TRICARE Service Center during operating hours. After the forms are submitted, patients should continue to be seen at their current clinic until Screaming Eagle Medical Home staff contact them to meet their personal healthcare team.

Active duty Family members who are currently enrolled in TRICARE Standard/Extra, or who have recently transferred from another TRICARE Region and live in the zip codes listed above may enroll to the Screaming Eagle Medical Home Clinic by completing a TRICARE Prime Enrollment Application. Applications may be obtained at www.tricare.mil or at the TRICARE Service Center (address listed above).

Active Duty Family membersThe Screaming Eagle Medical Home is inspired from the ground up by patients through their feedback about Army healthcare. Those ideas and suggestions are embodied in the Patient-Centered Medical Home concept, a new model of healthcare being adopted across Military Healthcare.

“Patients partner with a team of healthcare providers to develop a comprehensive, personal healthcare plan,” said Screaming Eagle Medical Home Group Practice Manager Ken Russell. Patients are assigned to teams who take care of health issues as they arise, ensure delivery of prevention screening and services, manage chronic conditions and promote a spirit of health and wellness.
