BACH Screaming Eagle Medical Home staff will be available for questions at Rivers & Spires

Blanchfield Army Community Hospital LogoFort Campbell, KY – Staff members from Blanchfield Army Community Hospital’s Screaming Eagle Medical Home (SEMH) will host a booth on Franklin Street during Clarksville’s Rivers and Spires Festival. Active duty Family members can talk to SEMH staff Friday and Saturday, April 15th and 16th at the annual Clarksville festival. SEMH staff can explain services offered at the clinic and they will provide the paperwork needed to eligible Tricare beneficiaries who would like to enroll.

“This is a wonderful way to learn more about the services that are offered at the community based clinic and to meet some of the staff members who take care of our active duty Family population living in the Clarksville community,” said SEMH Group Practice Manager Ken Russell.

The SEMH, located next to Gateway Medical Center in Suite 301 of Medical Office Building One, is open for enrollment to active duty Family members living in the following zip codes: 37010, 37032, 37035, 37040, 37043, 37146, 42204, 42234 and 42265. Active duty Family members who live in the 37042 zip code and live within a 10 minute drive time from the clinic are also eligible to enroll.

Russell encourages anyone who has questions about the clinic to stop by their booth during the Rivers and Spires Festival on Friday and Saturday. “Our staff members are excited to share with you all the unique features offered by the clinic,” said Russell.

Eligible individuals interested in enrolling to the SEMH must complete and submit a Primary Care Manager Change Request Form. The form will be available at the Rivers and Spires Festival, the Tricare Service Center on Fort Campbell or may be downloaded from BACH’s website www.campbell.amedd.army.mil. Completed forms may be scanned and faxed to 931-431-6168, emailed to Kenneth.giffin@healthnet.com, taken or mailed to the Tricare Service Center, 2525 22nd Street, P.O. Box 444, Fort Campbell, KY 42223.
