Vaccination Requirements for Incoming Kindergartners and 7th Graders

The Clarksville-Montgomery County School SystemMontgomery County, TN – Parents of sixth graders and children who will enter kindergarten in August, should be aware of the immunization requirements needed to be admitted into school for the 2011-2012 school year.

Those entering kindergarten or 7th grade, will be required to have updated immunizations prior to entering school in the fall. It is important to note that it is state law that no child shall be permitted to attend school until proof of immunization is provided to the school. 

Tennessee state law has required facilities to accept only the Official Certificate of Immunization. Proof of immunization must be on the new certificate, not attached to it. Healthcare providers or the health department will provide parents with the certificate when the child is given the required immunizations. School staff, including the school nurse, cannot complete the certificate. This must be done by a healthcare provider. The school cannot accept a faxed copy of the certificate. The original certificate must be brought in at the time of registration.

The Montgomery County Health Department is open Monday through Friday, 8:00am – 4:30pm and will be open during the week of Spring Break, March 21st-25th. Walk-in appointments will be welcome to receive the required immunizations. The charge for the visit will be based on income and family size.
