Tennessee Bureau of InvestigationNashville, TN – An investigation by Special Agents with the Medicaid Fraud Control Unit of the Tennessee Bureau of Investigation (TBI) has resulted in the arrest of a Middle Tennessee registered nurse, on charges of patient abuse and reckless homicide.

In November 2018, working alongside the Vulnerable Adult Protective Investigative Team with the Davidson County District Attorney General’s Office, TBI Agents began investigating the circumstances surrounding the death of Charlene Murphey (DOB 07/21/1942).

TBI Charge Two Men with Theft from Assisted Living Center, Facility Residents

Tennessee Bureau of InvestigationNashville, TN – A joint investigation by Special Agents with the Tennessee Bureau of Investigation (TBI) and investigators with the Davidson County District Attorney General’s Office, with assistance from investigators from the Tennessee Comptroller of the Treasury, has resulted in the arrest of two Nashville men on theft charges stemming from their management of an assisted living center.

At the request of 20th District Attorney General Glenn Funk, in October 2017, TBI Special Agents joined investigators with the DA’s office in investigating allegations of misspending at the Autumn Hills Assisted Living Center.

Tennessee Bureau of Investigation arrests Former Caregiver charged for Identity Theft

 

Tennessee Bureau of InvestigationNashville, TN – An investigation by Special Agents with the Medicaid Fraud Control Unit of the Tennessee Bureau of Investigation (TBI) has resulted in the arrest of a Davidson County woman on a charge of identity theft.

On November 30th, 2015, TBI Special Agents began investigating Cassandra Chrisman. At that time, Chrisman worked as a caregiver for a home care service provider.

TBI Medicaid Fraud Investigation Indicts, Charges Nurse

 

Tennessee Bureau of InvestigationNashville, TN – Special Agents in the Tennessee Bureau of Investigation’s Medicaid Fraud Control Unit have obtained an indictment for a nurse accused of falsifying records, resulting in fraudulent claims to TennCare.

After receiving information from the Tennessee Department of Health, Agents began investigating Brenda Shields (DOB 4-4-70) on March 14th. During the course of the investigation, Agents developed information that Shields, while working as an Licensed Practical Nurse (LPN) for CareAll, falsified timesheets and nursing notes, forged client signatures, and did not work some of her shifts as reported to her employer.

Former Bank Manager nabbed in TBI Theft and Forgery Case

 

Tennessee Bureau of InvestigationNashville, TN – Special Agents from the Tennessee Bureau of Investigation (TBI) have obtained indictments for a former bank manager accused of misusing her position for financial benefit.

At the request of 20th District Attorney General Glenn Funk, TBI Agents began investigating Ashley Rubbert Comer (DOB 7-23-86) on May 20th, 2015. During the course of the investigation, Agents developed information that Comer, while working as the interim manager of NGH Credit Union, opened loans in the names of family and friends without their consent and accessed the funds for her personal use.

TBI indict Nashville Area Pastor, Associate in Tennessee Grant Money Theft Case

 

Tennessee Bureau of InvestigationNashville, TN – Special Agents from the Tennessee Bureau of Investigation have obtained indictments for a Nashville area pastor and his associate, accused of securing state grant money for an addiction treatment program that did not exist.

At the request of 20th District Attorney Glenn Funk, and with the assistance of the Tennessee Comptroller of the Treasury, TBI Agents began investigating Clinton Lewis and Andre Trice on June 15th, 2015.

Tennessee Bureau of Investigation indicts Macon County Woman on charges of Forgery, False Insurance Claims

 

Tennessee Bureau of InvestigationNashville, TN – An investigation by Special Agents with the Tennessee Bureau of Investigation (TBI) has resulted in the arrest of a Lafayette woman who is charged with multiple counts of forgery and filing false insurance claims.

At the request of 20th District Attorney General Glenn Funk, TBI Special Agents began investigating Amy Crawford on December 9th, 2014. Crawford worked as a medical assistant for Pinnacle Surgical Partners in Hermitage from December 2013 through June 2014.

TBI Misconduct Investigation gets Davidson County court officer fired, charged

 

Tennessee Bureau of InvestigationNashville, TN – Special Agents from the Tennessee Bureau of Investigation have obtained an indictment for a former Davidson County court officer accused of misconduct.

At the request of 20th District Attorney General Glenn Funk, TBI Special Agents began investigating Altavease McCluskey on April 8th. During the course of the investigation, Agents developed information that McCluskey, who worked as an electronic monitoring officer for the Davidson County court system, entered into a sexual relationship with a defendant she was supervising.

Tennessee Bureau of Investigation indicts Caretaker Accused of Abusing Special Needs Patient

 

Tennessee Bureau of InvestigationNashville, TN – The Tennessee Bureau of Investigation (TBI) has obtained an indictment for a man accused of abusing an impaired man in his care.

On April 16th, with the assistance of the Department of Intellectual and Developmental Disabilities, TBI Agents began investigating Willie Miles, Jr., after receiving a referral from Adult Protective Services. Two days earlier, a man with severe developmental disabilities was admitted to Southern Hills Medical Center for breathing difficulties.
