Fort Campbell’s Blanchfield Army Community Hospital releases updated School and Sports Physicals Schedule

 

Medical Homes offering appointments weekdays, some Saturdays
Note: Young Eagle Medical Home previously offered school and sports physical appointments for August 8th, but that date has changed to August 15th.

– Blanchfield Army Community Hospital (BACH) patient-centered medical home teams are now providing school and sports physicals for BACH-enrolled TRICARE Prime beneficiaries’ during regular medical home hours Monday through Friday. In addition, BACH’s patient-centered medical homes are offering eight Saturday appointment dates.

Patients assigned to the Patient Centered Medical Homes listed below may begin scheduling school and sports physical appointments now. Weekday appointments are available during normal clinic hours, or you may schedule an appointment at your assigned medical home on one of the Saturday’s listed below.

All beneficiaries, including individuals enrolled to a network provider, may schedule an appointment for one of BACH’s School and Sports Physical Saturday clinics.

“School and sports physicals are essential for families to ensure their children meet the requirements for school attendance and sports participation. Even more importantly, it provides an opportunity for our medical homes to assess for appropriate growth, development and other health care maintenance needs,” said chief of primary care Col. David Brown.

“Routine appointments during the work week are available all summer, but we offer Saturday school and sports physical dates as a convenience to our working parents and to provide access to network prime enrollees for whom sports physicals are not a Tricare-covered benefit,” said Brown.

Patients seen by a BACH provider

BACH medical homes providing school and sports physicals include Byrd Family Medical Home, Air Assault Family Medical Home, Young Eagle Medical Home and the Screaming Eagle Medical Home. Patients assigned to a BACH medical home may book appointments by calling the appointment line at 270.798.4677 or 931.431.4677, visiting www.tricareonline.com, or by emailing your care team using RelayHealth, the Army’s Secure Email Messaging System.

Patients assigned to SEMH and enrolled to RelayHealth may book their appointments with SEMH staff directly using the secure email system. SEMH patients using the appointment line will be forwarded to the SEMH care team to schedule an appointment.

To help care teams make preparations for your visit when calling the appointment line or sending a RelayHealth message, please inform the team whether your child will attend a school in Kentucky or Tennessee, or another state during the 2015-16 school year. If you are leaving the Kentucky or Tennessee areas before school begins, please download the appropriate school form to take to your child’s appointment.

TRICARE Prime patients seen by a network provider

TRICARE prime beneficiaries assigned to a network medical provider may schedule an appointment with their network primary care manager (PCM) for school or sports physicals or they may call for an appointment to attend one of the School and Sports Physical Saturdays using BACH’s appointment line at 270.798.4677 or 931.431.4677.

What patients should bring to the school and sports physical appointment

To receive a school or sports physical, students must bring their immunization record (shot record) to the appointment. The BACH patient-centered medical home teams offer walk-in hours for immunization services to BACH enrolled patients from 8:00am to 3:30pm, Monday through Friday.

Immunizations may also be given before or after the school or sports physical examination is scheduled. If other medical concerns arise during the school or sports physical examination, a follow-up appointment should be scheduled with your PCM in order to ensure proper time and attention is devoted to the medical need.

Routine physical exams are valid for one year and are required for the following students:

  • Any child entering Christian County or Clarksville-Montgomery County schools for the first time
  • Any student participating in school organized sports
  • Any youth participating in Fort Campbell’s organized sport programs
  • Any adolescent who enrolls in the 6th grade in Christian County or Fort Campbell Schools
  • Any adolescent who is entering the 7th grade in Tennessee

If your child had a school physical completed at one of BACH’s Medical Homes within the last 12 months and has had no medical changes, it will not be necessary to receive another school physical.

If your child had a sports physical completed at one of BACH’s Medical Homes within the past 6 months and has had no medical changes, it will not be necessary to receive another sports physical.

BACH medical home patients who do not need a new physical may contact their care team to have the required school forms completed. Paperwork may take up to three business days to complete.

Patients unable to attend a scheduled school or sports physical appointment Monday through Friday should call the BACH appointment line to cancel at least two hours prior to the appointment. To cancel a Saturday appointment, please call the appointment line any time before noon on the Friday before your scheduled appointment.

Special notes: Please remember to bring each child’s school or sports physical examination forms and immunization records to the appointment. To avoid delays, BACH’s care team requests that you complete all required forms prior to the appointment. Children are asked to wear shorts and a short-sleeved shirt to facilitate the examination.
